If you are interested in taking advantage of HowsYourHealth you must read the instructions below and complete the registration. Within a month after you have registered you should:

1. view the brief patient video about using HowsYourHealth;
2. customize it for your organization,
3. test HowsYourHealth on at least 10 patients and up to 75 patients for free, and
4. examine ways to implement easily and broadly.

Within a month you will be contacted by email with specific information for organizations like yours. You will also be offered an optional conference call to discuss any questions you might have.
